Fine Arts & Crafts Festival and download the Insurance Form to complete and email to PutnamCTArtsCouncil@gmail.com.   

As we get closer to the event, you will be getting additional information about your booth number, parking, etc...

Accepted Artists must also complete the Insurance form to the right. After completing payment, return to this page and download the form Email the completed form to PutnamCTArtsCouncil@Gmail.com

with "<your name> Insurance Form" in the subject
